Case Management
With the assistance of a case manager, the consumer and his/her family members develop an Individualized Plan that identifies training and services tailored to the individual’s specific needs. These may include assistance with money management, transportation, or home management skills; learning social and communication skills or healthier lifestyle and leisure services.
Residential Services
We offer adults a variety of residential options based on his/her needs and level of independence. Consumers may live in an integrated residential home with round-the-clock supervision by CBE staff; in a host home (similar to foster care) or in a group home if they require specialized physical/medical attention. Each residential program is designed to further the independence of each person and address special needs. These services may be offered directly or are available through a service organization.
