Colorado Bluesky Programs and Services
Home and Community-Based Services (HCBS) are made available by the State of Colorado through several funding Programs overseen by the Division of Intellectual and Developmental Disabilities (DIDD), and administered by the CCB. These Programs are funded with Medicaid (also known as a "Medicaid Waiver") and State funds, and are available for eligible adults and children in Colorado. Services within each type of program are available to eligible, enrolled individuals based upon the identification and prioritization of individual needs. Services to address the identified needs are purchased from approved contractors and Program Approved Service Agencies, using Program funds available and as authorized through the individual's Plan. Early Intervention Program (ages Birth - up to age 3)
Our Early Intervention program serves families who have a child under three years old who has a significant delay in their development. We partner with our local school districts to determine eligibility for the program and then deliver services in the child"s home. Children With Autism Waiver (ages Birth - up to age 6)
The Home and Community Based Services Waiver for Children with Autism (HCBS-CWA) is a program for children ages birth to 6 years that have a medical diagnosis of Autism. The intent of the Autism Waiver program is to provide intensive behavioral supports and services to young children with Autism. The Autism Waiver allows children access to Medicaid services who may not otherwise qualify for Medicaid State Plan Benefits as well as providing funding to purchase specific services which are not covered under the Medicaid State Plan. Children's Extensive Support Waiver (Birth - up to age 18)
Children's Extensive Support (CES) services are provided through the Medicaid Home and Community Based Services (HCBS) CES Waiver for the most challenging children with developmental disabilities. The CES Waiver allows children access to Medicaid services who may not otherwise qualify for Medicaid State Plan Benefits. These children continue to reside in the community with their family. The services allowed include personal assistance with daily living, respite, homemaking skills, day services, assistive technology, home modification, professional services, transportation and emergency assistance.
Family Support Program
The Family Support Program is a state funded program to provide individualized supports to families who are caring for a family member with developmental disabilities/delays, including connecting them with resources and providing limited financial help.
Adult Supported Living Waiver (ages 18+)
Supported living services (SLS) are provided through the Medicaid Home and Community Based Services (HCBS) SLS Waiver. This program provides a variety of services and supports for adults living in their own homes or with family in the community. The services allowed include personal assistance with daily living, homemaking skills, employment, day services, assistive technology, home modification, professional services, transportation and emergency assistance.
Adult Residential and Day Services (HCBS-DD) Waiver (ages 18+)
Residential services in settings that include group homes, host homes, and apartment homes provide 24/7 support. Support for each individual is designed according to their need. Day Services furnished 4 or more hours per day in a community-based setting, encompassing both health and social services needed to ensure the optimal functioning of the participant
Community Integrated Employment Services (ages 18+)
Provides customized support to enable an individual to secure and maintain employment in the community. Employment Services may be available for adults enrolled into programs, or possibly those on adult waiting lists.
